The Palliative Coach
A team of nurse practitioners providing telehealth palliative care to aid the older adult and their caregivers through their health journey. We accept insurance. We focus on symptom management, care coordination with their other providers to improve their quality of life. We are different than hospice, you do not need to stop any treatment to take advantage of this medicare benefit.
